FUNCTIONAL MEDICINE PATIENTSBefore your first appointment you will schedule a paid consultation with Nurse Practitioner Abby to discuss your health needs.After your consultation you will be emailed a Comprehensive Health History form to fill out along with other new patient paperwork. You will also be asked to send us the last 3 years of your medical records pertaining to the condition for which you are seeking care. The first appointment is scheduled after we receive your new patient paperwork and medical records. ​When your appointment is scheduled you will be asked to pay $188.00. This covers the cost of an extensive review of your history and medical records. Your medical records and history will not be reviewed until this down payment is made. In order to get to know you and your story the first visit is 1 hour long. This visit includes a physical exam, report on the analysis of your medical records and health history questionnaire and instructions on any needed labwork. The second visit is 45 minutes to 1 hour long. This visit includes analysis of labwork if any were done and presentation of your personalized plan of care. Follow up visits are 30 minutes long and include an evaluation of your personalized plan of care.

We see patients from 18 years old to seniors

At this time we are not accepting new Primary Care patients. Patients who are seeing us for Functional Medicine must have and keep a close relationship with their primary care providers while being seen at our office.

Labwork that requires blood draws can be done at a lab center or at our office. Many of the labs can be done at home and shipped to the lab company directly by the patient.

In order to provide you with personalized care we ask that your initial visit be in the office. We are happy to provide you with Telehealth visits if needed after your first appointment.

COSTS FOR FUNCTIONAL MEDICINE SERVICESConsultation Cost: $50Visit Costs: Initial visit is $376.00. The $188.00 paid at time of scheduling is a part of the initial visit cost. After the initial visit you pay for the first three months of visits which is $833.00. The next 3 months of visits cost is $510.00. These do not include lab tests or supplement cost.
