What can I expect at my appointments?

FUNCTIONAL MEDICINE PATIENTS

Before your first appointment you will schedule a paid consultation  with Nurse Practitioner Abby to discuss your health needs.

After your consultation you will be emailed a Comprehensive Health History form to fill out along with other new patient paperwork. You will also be asked to send  us the last 3 years of your medical records pertaining to the condition for which you are seeking care. The first appointment is scheduled after we receive your new patient paperwork and medical records.

When your appointment is scheduled you will be asked to pay $188.00. This covers the cost of  an extensive review of your history and medical records. Your medical records and history will not be reviewed until this down payment is made. 
 
In order to get to know you and your story the first visit is 1 to 1.5 hours long. This visit includes a physical exam, report on the analysis of your medical records and health history questionnaire and instructions on any needed labwork.
 
The second visit is 45 minutes to 1 hour long. This visit includes analysis of labwork and presentation of your personalized plan of care. 
 
Follow up visits are 30 minutes long and include an evaluation of  your personalized plan of care.
 
PRIMARY CARE PATIENTS

If your appointment is for a condition that just started you will be scheduled to see the Practitioner as soon as possible. Medical records will be requested after the time of the appointment.  Payment is expected at the time of service.

If your appointment is for a condition that has been going on for a  while and is not causing problems eg. high cholesterol, annual exam etc. we will schedule you for 3 weeks out. This will allow you time to request the last 3 years of your medical records from your other providers and the Nurse Practitioner time to review them before your visit. Payment is expected at the time of service.

Do you see patients of all ages?

We see patients from 12 years old to seniors.

Do you provide Primary Care Services?

At this time we are not accepting  new Primary Care patients. 
Patients  who are seeing us for  Functional Medicine must  have and keep a close relationship with their primary care providers while being seen at our office.

Question 4

Where do I get lab work done if needed?

Accepted Answer

Labwork that requires blood draws can be done at a lab center or at our office. Many of the labs can be done at home and shipped to the lab company directly by the patient.

Do you offer TeleHealth visits?

In order to provide you with personalized care we ask that your initial visit be in the office. We are happy to provide you with Telehealth visits if needed after your first appointment.

What are the costs for your services?

COSTS FOR FUNCTIONAL MEDICINE SERVICES
 • Consultation Visit: $50
 • First Visit: $376.00. This includes the $188.00 down payment at time of appointment scheduling. This does not include cost of lab tests that may be ordered.
 • Follow-up Visits (paid 2-3 months in advance): This payment covers all office visits, supplements, and repeat lab work if needed for 2- 3 months. The cost varies based on the plan of care. This will be discussed in more detail at the consultation visit.

COSTS FOR PRIMARY CARE SERVICES
ADULT - 18 years and older
 • Annual Exam: $206.00
 • Initial Office Visit: $206.00
 • Follow-up Visit: starting at $155.00*

CHILD - 12-17 years old
 • Annual Exam: $155.00
 • Initial Office Visit: $155.0
 • Follow-up Visit: starting at $129.00*
 • Sports Physicals: $77.25

Payment plans are available for all visits.
Payment options: Care Credit, credit card, check, cash, or Health Savings Account Card

*Prices are subject to increase based on the number of diagnoses and complexity of care given.*

Do you accept insurance? What about payment plans?

Our office does not accept insurance for office visits.

We offer payment plans to make getting healthier more affordable for you. You can pay with credit card, cash, check or a Health Savings Account.
